If your child has a dietary restriction, our cafeteria can accommodate it. Please see the Food Services page on the district website for more information. Please also let me and your child’s teacher know.
If your child has a chronic health condition, please notify me and your child’s teacher. Accommodations are available for those who need them.
AZ state law mandates that all children are fully immunized or have a valid exemption on file prior to attending school. A vaccination series is required for preschool attendance, again at 4 years old, and another at 11 years old. Please provide an updated copy of your child’s shot record after receiving vaccinations.
All medications must be brought to the health office by a parent or guardian in the original labeled container or prescription bottle. The label must include dosing for the age/weight of your child, or a prescription is required. Prior written parental consent is required for administration at school.
If your child must carry life-saving medications such as an inhaler or epinephrine, please contact me for the appropriate consent to self-carry. A child carrying medications without permission is subject to disciplinary action according to school regulations because it’s a safety/poisoning hazard for all children.
If your child needs Tylenol or ibuprofen for an extended period of time, please bring a bottle in the original labeled container to the health office. Prior written parental consent is required for administration at school. The consent form is also available at the online registration platform.
Our playground is located on a hill without grass. We use soft-fall under equipment. Outdoor recess is dependent upon weather conditions. Please dress your child in appropriate shoes for the terrain, avoiding flip-flops, high heels, and slippers.
We keep a small supply of donated clothing for bathroom accidents. (Thank you!)
Emergencies always take priority. Students in a non-emergent condition may be asked to return later so more urgent cases can be handled appropriately. Students are sent home according to FUSD policies and nursing protocols. Please update your contact information promptly and indicate your order of contact in ParentVUE. We cannot keep sick or injured children at school for extended periods of time. Please plan to pick up your child within the hour.
Children who do not need to be sent home, but require further observation, will receive a note or head injury report to bring home to their parents.
For resolved issues, a copy of the annotated triplicate pass, used to refer your child to the nurse, is always sent home (i.e. transient aches and pains, bathroom accidents, etc.).
Minor issues are often handled in the classroom.
Please check your child’s backpack daily for notes and passes. Courtesy calls home for non-emergencies are only conducted if no students are waiting for nursing care and time allows.
Please keep your child home when ill, both to allow adequate time to recover and to prevent the spread of illness. If you aren’t sure whether to send your child to school, please call me at 928-773-4122.
Your child should stay home for the following reasons:
Communicable disease, such as chicken pox, shingles, strep throat, flu, COVID-19, etc. (Please call to let me know.)
Fever/chills of 100.4°F or higher within the past 24 hours, with or without medication.
Vomiting or diarrhea in the past 24 hours.
Started antibiotics within the past 24 hours.
Heavy congestion or constant cough, with or without fever.
Unexplained or spreading rash, with or without fever.
Draining or crusted, pink eyes.
Sore throat with white spots on the tonsils, or red spots on the palate, with or without fever (needs to see pediatrician before returning to school).
Uncontrolled, severe pain.
Extreme fatigue.
Untreated lice infestation. FUSD follows CDC recommendations regarding management of head lice.
